We know your time is limited. Add your name and a contact number, attach the referral, and we’ll do the rest: contact the patient, book them in and keep you updated.
What you can expect from us
- Prompt contact with your patient
- Reports after the initial and final consultations for Medicare referrals, and updates as required for other schemes
- Clinic appointments Monday to Friday (after hours and weekends by request), plus home visits and telehealth where needed
- Medicare CCM, DVA, NDIS (registered provider), workers compensation, CTP, Support at Home and private

Medicare CCM
Referral under a GP Chronic Condition Management Plan (or a pre-July 2025 GPMP/TCA, valid until 30 June 2027).
Workers comp & CTP
Please include the claim number and insurer. We’ll submit the treatment request.
NDIS & Support at Home
Please include plan management type, plan manager or provider details and goals where possible.
